The experienced, skilled professionals of Olathe Medical Center's Home Health Services are available to provide health care when a medical situation arises making it difficult to leave home. Services are continued until the patient reaches his or her optimum level of independent functioning.

If you are homebound and have a need for skilled nursing or rehabilitative therapy, Home Health can assist you to recover in the comfort of your home. You must be under the care of a physician and he/she must submit an order for our services.

The Home Health service area includes Johnson, Douglas, Miami, Franklin and Wyandotte counties in Kansas and is bounded by:

• I-70 on the north,
• Eudora and Baldwin on the west,
• State Line Rd on the east and
• Ottawa and Osawatomie on the south.

Home Health is typically reimbursed through Medicare, Medicaid, private insurance or private pay. Limited resources are available for indigent care. Services are provided to all persons without regard to race, creed, national origin, handicap or age.

Care Givers
Your patient(s) might benefit from Home Health Services when:

• This is the 3rd or 4th admission in the past year
  • i.e. report of multiple patient falls recently
• Significant changes or additions to medications have occurred during this facility stay
• There is an increase of severity of disease or new symptoms
  • uncontrolled blood sugars, diabetic patient
  • exacerbation of respiratory problems, COPD patient
• A new diagnosis is added to existing problems
• Post-operative care/instruction/monitoring would be helpful in the patient's home setting
• Physical, occupational or speech therapy is needed
• New daily procedures as a result of recent hospitalization:
  • blood pressure/blood sugar monitoring
  • wound care
  • infusion care
  • stoma care
