Admission
Considerations
- Patient is under
the care of attending M.D.
- Patient agrees
to Hospice care
- Patient has
a confirmed life-threatening diagnosis
- Life expectancy
approximation six months to one year
- Patient must
have a primary caregiver, this person does not have to be a family
member
- Care is provided
in the home, in a nursing home or an assisted living facility
- DNR (Do Not
Resuscitate) is not mandatory for admission
- DNR forms are
offered to all patients
- Hospice promotes
comfort and quality days, it does not support measures to prolong
life
Services
Provided
Hospice care is
provided in the comfort of your own home by a team of health care
professionals including:
- Registered nurses
- Licensed social workers
- Spiritual counselors
- Home health aide
- Registered dietician
- Volunteer
- Therapy professionals if appropriate
- Other services available include:
- Respite care (to relieve caregivers for up to 5 days at a
time)
- Services to nursing facility residents
- Short tern, more intensive home care support by hospice professionals
when indicated
- Grief and loss support (group and individual counseling available)

* For patients
meeting criteria, these benefits include 100% coverage for medical
equipment, supplies and medication that are related to the life-threatening
